be.About the role and where you will be working:The Dementia
Respite Project Coordinator, in consultation with the Respite
Centre Co-Ordinator, focuses on improving access to initial respite
care services for people with dementia and their carers. There is a
particular focus on the quality, experience and innovative
approaches to improving respite care planning for people living
with dementia and their carers. This position is responsible for
understanding, reporting and ensuring the successful execution of a
Government funded grant.We'll be asking you to: -Develop a model of
care applying the principles of the 'Staying at Home' program
guidelines developed by HammondCare, ensuring these models improve
the experience and quality of respite for people living with
dementia and their carersEnsure people living with dementia stay at
home longer and entry to permanent residential care is
delayedProvide regular feedback to internal and external
stakeholders about project progress, successes and barriers for
successAbout YouYou are a compassionate and highly organised
individual with a strong understanding of the needs of people
living with dementia and their carers. You bring experience in
project coordination, ideally within health, aged care, or
community services, and have a passion for improving service
accessibility and quality. Your collaborative approach enables you
to work effectively with a range of stakeholders, and you are
confident in grant management, reporting, and delivering project
outcomes within defined timeframes. Innovation, empathy, and a
commitment to person-centred care are at the heart of your work.To
be considered for this role, you'll need:-Demonstrated ability to
follow risk management strategies, identify and report hazards and
adhere to work health and safety policies and
proceduresWell-developed verbal and written communication
skillsDemonstrated ability to gather, compile and analyse project
data and report within nominated timeframesDemonstrated
understanding of Dementia and the journey experienced by the person
living with Dementia and their carer/sDemonstrated ability to work
within nominated timeframes, meet performance indicators and
deliver operational outcomesDemonstrated ability to work within a
nominated budget to achieve operational outcomesWorking with us:At
