As a key member of our integrated healthcare services, you will have the opportunity to work in diverse settings, including facility-based and home-based environments, focusing on holistic patient-centered care. This role requires a strong background in physical therapy, excellent patient communication, and a commitment to delivering exceptional care. Assess patient condition by conducting physical examinations; establishing treatment plans. Implement physical therapy treatments to improve mobility, relieve pain, and prevent or limit permanent physical disabilities in patients. Maintain patient records by documenting their condition and treatment plans. Promote patient health and well-being through the application of physiologic and therapeutic interventions. Educate patients and family members about appropriate physical therapy methods and techniques. Maintain professional and technical knowledge by attending educational workshops; review professional publications; establish personal networks. Proven ability to diagnose and manage motion and pain issues Strong interpersonal and communication skills Skills in handling complex patient situations with a calm and rational approach Expertise in home health care and outpatient therapy Ability to manage time effectively and make autonomous decisions related to patient care Medical Insurance Dental Insurance Vision Insurance 401k Current Physical Therapist license CPR certification Mandatory continuing education credits as per state and national regulations
