Job Description Job Description Description: Job Title: Hospice RN Case Manager Job Summary: We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and support for patients and their families during end-of-life care. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. Key Responsibilities: - Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs. - Develop and implement individualized care plans in collaboration with the interdisciplinary team. - Provide direct nursing care, including medication administration, symptom management, and wound care. - Educate patients and families about the hospice process, available resources, and care options. - Coordinate care with other healthcare providers, ensuring continuity and quality of care. - Document patient progress and maintain accurate medical records in compliance with regulations. - Participate in team meetings and contribute to quality improvement initiatives. Skills and Qualifications: - Current Registered Nurse (RN) license in the state of practice. - Bachelor of Science in Nursing (BSN) preferred. - Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care. - Strong assessment and critical thinking skills. - Excellent communication and interpersonal skills. - Ability to work collaboratively in a team-oriented environment. - Compassionate and empathetic approach to patient care. - Knowledge of hospice regulations and standards of practice. Requirements: Current, unencumbered Virginia RN license One year of nursing experience, preferably in hospice, home health, or skilled nursing Current Basic Life Support (CPR) certification Proficient in English (spoken and written) Excellent verbal and written communication skills Ability to meet deadlines and work independently Capable of performing CPR and airway obstruction relief Able to walk or stand for at least 4 hours and climb two flights of stairs Valid driver’s license with a satisfactory driving record and proof of auto insurance Must complete 12 hours of continuing education, annually
